This program has been designed to specifically support people with cancer, mental health and chronic conditions through individualised virtual care program. The program is based on evidence-based research, client and clinician feedback this program may include access to online resources, podcasts, as well as referral to individual counselling support, if required.

Will my personal clinical information be kept confidential?
